Please refer to the &#8220;Dates at a Glance&#8221; page in the Registration Guide for fee waiver registration dates. If a student pre-registers for classes before the fee waiver registration date, the waiver cannot be applied to those classes.<\/li>\n\n\n\n<li>Fee Waivers are accepted for one (1) course per part-of-term (up to four credit hours) for a maximum of four courses per academic year (fall\/spring\/summer). An employee may waive two (2) courses during the term as long as the course sessions (instruction periods) do not overlap. A separate form is required for each semester and summer course and must be submitted to the Business Office before the fee payment deadlines.<\/li>\n\n\n\n<li>Discounts and waivers cannot be used in conjunction with any other type of fee waiver or discount program.&nbsp;<\/li>\n\n\n\n<li>A separate form must be submitted each semester to verify continued employment\/eligibility and the employer certification signature should be obtained no earlier than 30 days before the fee payment deadline for the semester.&nbsp; <strong>Signatures may be obtained and submitted beginning July 1 for the fall semester, November 1 for spring, and April 1 for summer.<\/strong><\/li>\n\n\n\n<li>Teacher discount forms should be certified and display school or superintendent seal or stamp.<\/li>\n\n\n\n<li>The employee&#8217;s employment status and the dependent&#8217;s age on the first day of the term shall be used to determine eligibility for all discount programs.<\/li>\n<\/ul>\n\n\n\n<div style=\"height:15px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p class=\"wp-block-paragraph\">You may email the completed form to&nbsp;<a href=\"mailto:bursarmt@mtsu.edu\">bursarmt@mtsu.edu<\/a>.&nbsp; Please&nbsp;<strong>do not<\/strong>&nbsp;include social security numbers if you chose to email a copy of the form.&nbsp; Please use the student&#8217;s 糖心Vlog官方 ID number.&nbsp; If you have any questions, please email&nbsp;<a href=\"mailto:bursarmt@mtsu.edu\">bursarmt@mtsu.edu<\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Completed forms may also be mailed to or hand-delivered to the following address:<br>糖心Vlog官方 Business Office<br>290 Student Services and Admissions Center<br>1301 E Main Street<br>Murfreesboro, TN 37132<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">糖心Vlog官方 will not accept any discount or waiver form after the last day of final exams for the semester.
